How can Foundations for Change help your practice?

Are you a clinician in Arizona?  This blog is for you!  By now you may have heard about Foundations For Change… we’re the practice in Child/Family and Adult Psychiatric Services provider in the Phoenix metro area with the adorable dogs!  Our practices features a wide variety of options to help your patients find excellent care, in a seamless transition from you to us.  We were founded by Jeff Edelman, a veteran with combat experience who has a passion for child, family, and adult psychiatric services, veteran and law-enforcement care, and specialty focuses like anxiety, OCD, and eating disorders.  

Jeff is a board-certified Family Psychiatric Mental Health Nurse Practitioner with a second board certification in Child and Adolescent Psychiatric Mental Health. Foundations for Changes offers a full complement of psychiatric services that include evaluations and diagnosis, consultations with other providers and schools, medication evaluations and management in order to provide the highest quality of care to children, adolescents, military, first responders and their families.  Jeff’s experience with SMI and including Crisis Management, Assessment, and Treatment of PTSD within a first responder context gives him a unique ability to connect with clients who other practices may find extremely challenging.  

Our practice philosophy

We differ from other psychiatric practices because of our practice philosophy.  Jeff Edelman has spent considerable time and resources to develop a sister company, Foundations for Wellness, which provides high-quality hand-made CBD products as tools for emotional therapy, pain management, and more.  Jeff uses his experience and time-tested courses of treatment, but also looks at the patient’s individual needs; sometimes this doesn’t just mean prescribing a course of medication, but rather looking into alternative interventions.  One of these is ketamine therapy, and another is Neurostar ® TMS (transcranial magnetic stimulation) Therapy.  Our TMS treatment is non-invasive, and is typically covered by insurance.  Patients can expect that the typical treatment course will consist of 5 treatments per week over a 4 – 6 week period totaling 20 to 30 sessions.  The exact number of treatments each patient receives will be discussed and determined by the psychiatric provider.

Providers, if you refer a client to us, we assure you that we will communicate and collaborate among all parties which as you well know, is key to a successful course of treatment.  We are a fee-for-service practice but do take most major insurance plans, as well as cash, credit, debit, and payment plans.  Our offices are conveniently located in the  Glendale/Arrowhead area at 16155 N. 83rd Ave., Ste 211, Peoria, AZ 85382 less than 1 mile away from the Mariners/Padres baseball stadium, and our Phoenix/Metro Center area office off Dunlap and the I-17 highway at 2400 W. Dunlap Ave., Ste 124, Phoenix, AZ 85021.
